  • 2017 cost:  $110 (Pine City Clinic – FirstLight Hospital And Clinics 2017 cost:  $110 (cash, check or credit cards are accepted, payment is expected prior to the exam and insurance does not cover this)
  • The charge includes a physical exam and urine screen, if additional testing is required, there will be a separate charge for those services.
  • Our office charges $40.00 for the DOT follow up appointment to re-evaluate the certification.  Again, payment is expected prior to the exam, and insurance does not cover it.

Driving WEST-bound, East of Hinckley past Toby’s prior to Grand Casino.
75 miles south of Duluth. Take exit 183 for MN-48. Turn left onto Fire Monument Rd. Take a right onto Morris Ave, then a take left onto Sandy Lane.

Driving NORTH-bound, 80 miles north of the Twin Cities. Take exit 183 for MN-48. Turn right onto Fire Monument Rd. Take a right onto Morris Ave and then take a left onto Sandy Lane.
Driving EAST-bound, 20 miles east of Mora. Take Hwy 23 and turn left onto I-35 N. Turn right onto Fire Monument Rd. Take a right onto Morris Ave, and then take a left onto Sandy Lane.
