Patient Billing Information
It is our goal to process your healthcare bill correctly and in a timely manner. Our staff will file claims for you with your health insurance carrier or other programs such as Medicare and Medicaid. We need your help to collect needed information and other requirements to obtain coverage or assistance. Please bring your insurance card and any authorizations required under your health plan when you come to FirstLight.

After your insurance company pays us, we will send you a statement about any remaining balance you may owe. Insurance billing specialists are available to help you with any billing questions. Payment in full is due upon receipt of your statement. You may receive separate bills for physician services, such as professional interpretation by a radiologist or specialty physician consultation. These charges are not billed by our facility. If you have questions about these charges, please call the telephone number listed on that bill.

Payment Arrangements
If you are unable to pay your balance in full, please contact a patient account representative immediately to make payment arrangements. You may call us at 320-225-3340 or toll-free at 800-245-5671 to set up payment arrangements.

Financial Assistance
Financial aid will be offered to insured patients that meet the criteria outlined in our financial assistance policy. An application may be picked up in the business office or can be mailed to you upon request. Proof of income will be required to determine if you qualify. A portion or the entire amount of your charge may qualify for this write off.

FirstCare Program

To be considered for financial assistance through the FirstLight FirstCare Program, please review and complete the FirstLight FirstCare Program Forms.

The Application Form is fillable PDF and may be completed online (with the exception of signatures) and printed. Once printed, please sign and bring to FirstLight at the time of your next appointment.  Or if you prefer, you may scan the completed, signed form and email to the FirstLight business office at

Uninsured Discount
A discount will be automatically provided for medically necessary services to any patient without health insurance.

Full Charge Master Data File

January 1, 2019 a new Centers for Medicare and Medicaid Services (CMS) rule went into effect requiring  all hospitals to post their charge master file.  This list contains a charge description, actual charge and CPT code as required. Click here to view.

Clinic charges over $25

Effective July 1, 2019, a new law enacted by the Minnesota Legislature went into effect requiring all primary care clinics to post their top 25 procedures costing over $25.  The list contains our average commercial, Medicare and Medicaid payer reimbursement.  This list will be updated on an annual basis.  Click here to view excel file.
