At FirstLight Heath System, our Social Workers advocate for our patients and their families, by supporting, nurturing, and guiding them to meet the emotional and social needs essential to one’s well being, both while hospitalized and in the outpatient setting.

FirstLight’s social services providers work closely with the providers, nurses, and all other members within our healthcare team to figure out what is best for you, the patient.

The Social Workers at FirstLight are able to help you in a variety of ways:

  • Counseling for patients, families and caregivers
  • Crisis referral and intervention for mental health access
  • Discharge planning
  • Education and Support Groups — Cancer Support Group & Caregivers Support Groups
  • Access and information provided for
    • Home Care
    • Hospice
    • Housing
    • Long-term Care Counseling
    • Meals-on-Wheels
    • Medical Equipment
    • Palliative Care
  • Nursing Home/ Assisted Living transitions
  • Provide information on Health Care Directives
  • Provide referrals for community services

FirstLight was certified as a Health Care Home by the Minnesota Department of Health in June of 2016. This type of care is a team approach to health care by working in partnership with patients and caregivers to plan, coordinate care and provide continuous access.
